1. Were under the age of 31 as of June 15, 2012;
2. Came to the United States before reaching your 16th birthday;

____ Proof of presence in the U.S. on June 15, 2012 (bank or credit card statements, phone bills, date-stamped photos, affidavits from school &/or employer)
___ Check or money order payable to USCIS for $465.00
___ Passport-size photographs (probably 2)
